Housing Quality Standards (HQS) inspection and you need to certify that the failed items have been corrected.
Fill out the form, sign it, and download a completed copy for your records.
Deadline to submit
Submit this self-certification to section8landlord@phoenix.gov within 21 calendar days of the inspection date.
What you will complete on the form
- Owner/Landlord name (print)
- Unit address (unit number, city, state, ZIP)
- Date of the failed inspection

A list of the failed items from the Failed Inspection Notice (up to five lines), confirming each one has been corrected
Recommended supporting documents
- Photos showing the repairs are completed
- Invoices and/or receipts for completed work
What happens after submission

Arizona Quadel may conduct random confirmatory inspections on units that submit an Inspection Self-Certification.

Signatures
- Owner signature and date are required, along with the owner’s printed name.
- Participant signature may be listed as not required during COVID-19 (if applicable on the form).
