Use the City of Phoenix Housing Choice Voucher (Section 8) Interim Change Report to report changes that may affect your assistance,
including income, child care expenses, and family/household composition.
You can fill out the form, sign, and download a completed copy for your records before submitting it.
Important rules

You must report changes in family composition and income within 30 calendar days of the change.

No change will be processed unless the Interim Change Report is completed and returned with all required verifications.

Updates to your rental portion generally become effective the 1st day of the month after the program receives all necessary verifications.

Examples of changes you should report
- A household member is added (birth, adoption, or court-awarded custody)
- A household member is leaving or has left the unit
- Family break-up or other household composition changes
Section A: Change in income
- Select whether the change is an increase or decrease.

Provide the family member name(s), the source of income, hire/award date, previous amount, and new amount.

Attach verification documents (examples may include check stubs, award letters, or employer termination letters).
Verification is required before the change can be processed.
Section B: Change in child care expenses
- Select whether the change is an increase or decrease.
- Provide the child care provider’s name and contact information.
Section C: Change in family composition

Any addition to the household must be approved by the City of Phoenix Section 8 office
before the person moves into the unit.

All adults must sign an Authorization for the Release of Information form and must pass a background screening.

List each person being added or removed with legal name, current address, date of birth, relationship, and select Add or Remove.
Signature and certification
- Enter the last four digits of the head of household’s Social Security Number.
- Sign and date to certify the information is true and correct.
