Use this City of Phoenix Housing Choice Voucher Program form to request a rent increase for a
Section 8 (HCV) assisted unit. Complete the request, include the required attachments, sign,
and download a copy for your records before submitting.
Before you submit (important requirements)
- Timing: Submit your request in writing at least 60 days before the new rent effective date.
- Tenant notice required: Include a copy of the rent-increase notice you provided to the tenant or the request may not be processed.
- Limit: Only one rent increase per year will be accepted and approved.
- Attach the breakdown: Complete and submit the Breakdown of Rent and Miscellaneous Taxes/Fees with your request.
What you will fill out on the rent increase request
- Date
- Owner name, contact email, and contact phone
- Current rent, new requested rent, and new rent effective date
- Tenant name and unit address
- Unit details: number of bedrooms, bathrooms, and square footage
Amenities (helps with rent comparables)
The form includes an amenities checklist to help the Housing Department compare your requested rent to similar market units.
Select all that apply (examples include dishwasher, washer/dryer or hookups, gated community, pool, carport/parking, patio/porch,
ceiling fans, landscaping, pest control, and more).
Unassisted unit comparables (if applicable)
If your complex has market-rate (unassisted) units, provide information for your three most recently leased
unassisted units that are the same unit size as the unit you are requesting the increase for. Include the
address/unit number, date rented, and rent amount.
Utilities responsibility question
Indicate whether any utility responsibilities have changed from the original HAP contract (Yes/No).
Required certification (read before signing)
By signing, you certify the information is complete and accurate and acknowledge the Housing Department will evaluate the request by
comparing your requested rent to comparable market-rate units. The review may result in a denial, a decrease in the current rent, or an
approval of the requested increase. You also acknowledge the tenant’s portion may increase and the tenant may choose to move.
Breakdown of rent and miscellaneous taxes/fees (attached page)
Complete the attached breakdown to show how the total rent is made up. This includes:
- Base rent

RUBS (Water/Sewer/Trash) and any related tax, and whether RUBS is billed to the tenant separately
(if billed separately, those charges are not included in housing assistance payments)

Fees and taxes (mark whether each is required for all tenants): pet fee, pest control, garage/carport/parking, trash valet,
smart home, technology (internet/cable/phone), washer/dryer, PDL (renters insurance), and CAM (common area maintenance)
- Additional rent/other miscellaneous charges and related taxes (if any)
